Creatine May Do More Than Support Workouts

Creatine is usually associated with weight rooms, athletes and better exercise performance. But a new 12-week clinical study raises a different question: could creatine also support muscle and certain aspects of cognitive function in middle-aged and older adults who aren't exercising?

The findings are interesting, but they need to be put into perspective.

Researchers found that adults taking creatine monohydrate experienced increases in lean tissue mass, strength and muscular endurance, including people who did not participate in the study's exercise program. Some measures of cognitive function and memory also improved among people taking creatine. However, the cognitive findings were exploratory, and the study itself was relatively small.

That means the research does not show that creatine can replace exercise. Instead, it adds to the growing evidence that creatine may have uses beyond traditional sports nutrition.

The study included 64 adults who completed the 12-week intervention, all between 45 and 65 years old.

What Exactly Did the Researchers Study?

The research, published in the Journal of the International Society of Sports Nutrition, looked at the effects of creatine monohydrate in healthy, sedentary adults between 45 and 65.

The researchers initially enrolled 73 participants. Participants chose whether they wanted to take part in a non-exercise intervention or an exercise-and-diet intervention. Within those groups, they were then randomly assigned to receive either creatine monohydrate or a placebo.

Of those participants, 64 completed the 12-week intervention and were included in the main analysis. Their average age was approximately 54.5 years, and the group included 40 women and 24 men.

The creatine dose was 10 grams per day, divided into two 5-gram servings.

Researchers assessed participants at the beginning of the study, at six weeks and again at 12 weeks. They looked at several outcomes, including:

  • Body composition measured using DXA

  • Muscle strength

  • Muscular endurance

  • Blood markers

  • Cognitive tests

  • Questionnaires related to cognition and health

The exercise group also completed resistance and aerobic training three times per week, along with walking and an energy-deficit diet.

That distinction matters. The study wasn't simply comparing people who took creatine with people who took nothing. Instead, researchers looked at creatine in both an exercise setting and a non-exercise setting.

What Happened to Muscle Mass?

One of the more interesting findings involved lean tissue.

Participants taking creatine without the exercise intervention increased their lean tissue mass and showed improvements in certain measures of strength and muscular endurance. The exercise-plus-creatine group also gained lean tissue and experienced additional improvements in physical performance.

In the exercise and weight-loss group, researchers reported that creatine supplementation helped preserve and increase lean tissue while participants were following an energy-deficit diet.

This is relevant because maintaining muscle becomes increasingly important with age. Losing muscle and physical strength can make everyday activities harder and may contribute to greater vulnerability later in life.

But there's an important distinction to keep in mind:

The study does not demonstrate that creatine alone builds muscle as effectively as resistance training.

The greatest overall physical benefits were seen when creatine was combined with exercise and dietary intervention. The researchers concluded that creatine may help support muscle mass and strength, not that it should take the place of physical activity.

What About the Brain?

The study also looked at cognitive function, which is one reason the findings have attracted attention beyond the fitness world.

Researchers observed favorable changes in selected measures of cognitive function and memory among participants taking creatine, whether or not they exercised.

However, this part of the research needs to be interpreted carefully.

The authors classified the individual cognitive and biomarker findings as exploratory. They examined several different cognitive and biological outcomes, and the researchers noted that these findings need to be confirmed by future studies.

So it would be too early to call creatine a proven memory supplement or suggest that it prevents cognitive decline.

A more accurate way to look at the finding is:

This study provides preliminary human evidence that creatine supplementation may influence certain measures of cognition and memory, but larger and longer studies are needed.

That's very different from saying that creatine simply "boosts brain power."

Why Might Creatine Affect More Than Muscle?

Creatine is naturally produced by the body and plays a role in the system cells use to rapidly supply energy.

Most of the body's creatine is stored in skeletal muscle, but creatine is also found in the brain. Because brain cells have high energy demands, researchers have been interested in whether increasing creatine availability might influence certain aspects of brain function.

That provides a plausible biological reason for studying creatine beyond sports and exercise.

But a plausible mechanism is not the same as proof of a clinical benefit.

This study adds more human evidence, but it does not establish that creatine can prevent dementia, treat memory problems or improve cognitive health in everyone.

Does Creatine Work Without Exercise?

Based on this particular study, some benefits were observed even without structured exercise.

That's the part that stands out.

Participants in the non-exercise creatine group experienced increases in lean tissue mass and improvements in certain physical measures.

But saying creatine "works without exercise" can be misleading if taken too far.

Creatine is not a replacement for resistance training.

Exercise provides a mechanical stimulus to muscle, which is important for maintaining and developing strength and physical capacity. In this study, combining creatine with exercise produced greater overall physical benefits than taking creatine without exercise.

A better takeaway is:

Creatine may provide benefits even when someone isn't following a structured exercise program, but it appears to be more useful when combined with appropriate physical activity.

Is 10 Grams of Creatine a Normal Dose?

This is another point where readers should be careful about applying the study directly to themselves.

Participants received 10 grams of creatine monohydrate per day, divided into two 5-gram servings.

That's higher than the commonly used maintenance amount of around 3 to 5 grams per day discussed in creatine research. The researchers used the higher dose partly because of emerging research examining whether higher intake could influence brain creatine levels and cognitive outcomes.

Importantly, this study does not establish that 10 grams is the ideal dose for everyone.

The study also lasted only 12 weeks.

For that reason, people shouldn't interpret the findings as a universal recommendation to take 10 grams of creatine every day.

Is Creatine Safe?

Creatine monohydrate is one of the most extensively studied sports supplements, and research has generally found it to be well tolerated in healthy adults when used appropriately. A position statement from the International Society of Sports Nutrition concluded that available evidence does not show detrimental effects from short- or long-term creatine monohydrate use in otherwise healthy people.

Still, "generally safe" doesn't mean that creatine is appropriate for everyone.

People with kidney disease or other medical conditions, as well as those taking medications that could affect kidney function, should discuss supplementation with a healthcare professional before using creatine.

Creatine can also increase body weight, partly because it increases the amount of water stored within muscle. This isn't necessarily harmful, but it can be relevant for someone who is monitoring their body weight.

What This Study Cannot Tell Us

The results are interesting, but several limitations make it difficult to draw broad conclusions.

The study was small

Only 64 participants completed the intervention and were included in the analysis. That's useful evidence, but it isn't enough to determine how creatine might affect the much larger and more diverse population of middle-aged and older adults.

Participants chose their exercise group

Participants decided whether they wanted to take part in the exercise intervention rather than being randomly assigned to exercise or no exercise.

That makes it more difficult to determine whether differences between the exercise and non-exercise groups were entirely due to the intervention itself.

The study lasted only 12 weeks

Twelve weeks can show short-term changes, but it can't tell us whether those effects would continue for years.

Cognitive findings remain preliminary

The study examined multiple cognitive outcomes, and the authors emphasized that individual cognitive and biomarker findings should be considered exploratory. More research is needed to determine whether these changes can be reproduced and whether they are clinically meaningful.

The participants were a specific population

The participants were healthy, sedentary adults between 45 and 65. The findings should not automatically be applied to younger people, highly active athletes, people with significant medical conditions or much older adults.

What Should Readers Take From This?

The most useful interpretation isn't "you can take creatine instead of exercising."

It's this:

Creatine may be one tool that supports healthy aging, particularly when the goal is preserving muscle and physical function.

For someone who already exercises, the evidence supporting creatine for training-related performance and lean mass is considerably more established.

For someone who is sedentary, this newer study suggests there may still be potential benefits, but that doesn't eliminate the importance of becoming physically active.

If maintaining muscle is the goal, resistance training remains an important stimulus. Creatine may complement that effort rather than replace it.

And for anyone considering creatine specifically for cognitive benefits, expectations should remain conservative. The research is interesting, but it isn't yet strong enough to consider creatine a proven cognitive-health intervention.

The Bottom Line

A new 12-week study adds an interesting finding to the growing body of creatine research: creatine monohydrate was associated with improvements in lean tissue mass, strength and muscular endurance even among middle-aged and older adults who did not participate in the study's exercise program.

Some measures of cognition and memory also improved, but those findings were exploratory and still need confirmation.

The biggest lesson isn't that exercise is unnecessary.

Instead, the research suggests that creatine's potential usefulness may extend beyond the gym. For healthy adults interested in maintaining muscle and strength as they age, creatine could be one part of a broader strategy that also includes appropriate physical activity and good nutrition.

Anyone considering supplementation should take their individual health situation into account and discuss it with a qualified healthcare professional, particularly if they have kidney problems, other medical conditions or take medications.
