Fitness Tests After 65: What New Research Really Shows

Getting older does not automatically mean losing the ability to move well, stay independent, or remain physically active.

But researchers increasingly recognize that how well a person can perform everyday physical tasks may reveal something about overall health that body weight or appearance alone cannot show.

A large 2026 study published in JAMA Network Open examined whether objectively measured physical fitness was associated with the risk of death among adults aged 65 and older.

The researchers looked at several different aspects of fitness rather than treating fitness as a single number. Participants were assessed for cardiovascular endurance, muscular strength, flexibility, balance, and agility.

The study included 13,423 adults aged 65 years or older who completed standardized physical-fitness assessments in Taiwan between January 2015 and November 2016.

Rather than relying only on questionnaires asking whether someone was active, researchers used physical tests to measure different components of fitness.

The assessments covered four broad areas:

  • Cardiorespiratory fitness

  • Muscular strength

  • Flexibility

  • Balance and agility

This matters because two people can both describe themselves as "active" while having very different physical abilities.

One person might walk regularly but struggle to stand from a chair.

Another might have strong legs but poor balance.

1. Balance and Agility

Balance and agility showed some of the strongest associations with mortality in the study.

Participants completed a one-leg standing test and an 8-foot up-and-go test.

Why might balance matter?

Good balance can contribute to safer movement.

That is one reason functional fitness deserves attention even when someone does not consider themselves an "athlete."

2. Lower-Body Strength

The researchers also evaluated muscular strength using a 30-second chair-stand test.

Participants repeatedly rose from a seated position without using their arms.

Standing from a chair involves the hips and legs, but it also requires coordination and control.

The study was a cohort observational study.

Researchers observed relationships between physical-fitness performance and subsequent mortality.

They did not randomly assign participants to different exercise programs and then determine whether a particular fitness improvement caused longer life.

For example, people who performed better on physical tests may have differed from poorer-performing participants in other ways.

They may have had different levels of underlying health, socioeconomic circumstances, previous activity, or other characteristics.

The researchers also assessed fitness at a particular point in time rather than continuously tracking changes in physical ability throughout aging.

So the findings should be viewed as evidence that physical fitness is associated with health outcomes, rather than proof that a particular test determines how long someone will live.

Should You Perform These Tests at Home?

Not necessarily.

The assessments used in the research were standardized tests administered under controlled conditions.

Trying to reproduce them at home does not turn them into a medical screening tool.

For someone who is healthy, steady, and accustomed to physical activity, simple functional movements can be useful as personal benchmarks.

But safety comes first.

Anyone with a history of falls, significant balance problems, unexplained dizziness, substantial pain, a recent injury, or a medical condition that affects exercise safety should discuss appropriate activity with a healthcare professional before attempting challenging or timed tests.

There is no benefit to risking a fall simply to obtain a fitness score.

A More Useful Question Than "Do I Look Fit?"

Fitness is often discussed in terms of appearance.

But aging changes the priorities.

A person may have a healthy body weight and still have poor balance or limited functional strength.

Another person may not look particularly athletic but may have excellent endurance and mobility.

Instead of asking only:

"Do I look fit?"

Consider asking:

"Can I comfortably do the things I need and want to do?"

Can you get up from a chair?

Can you climb a flight of stairs?

Can you carry groceries?

Can you walk without becoming unusually exhausted?

Can you turn and change direction safely?

Can you maintain your balance?

Can you get down to the floor and back up safely if your daily life requires it?

These questions are much closer to the concept of functional fitness.

How to Build a Broader Fitness Base After 65

The study does not prescribe a specific exercise program.

However, its findings provide a useful reminder that older adults should not focus exclusively on one form of exercise.

A well-rounded routine can include several components.

Strength training

Resistance exercises can help maintain muscular strength and physical function.

This might involve bodyweight movements, resistance bands, machines, or free weights depending on ability and experience.

Aerobic activity

Walking, cycling, swimming, dancing, and other aerobic activities can help maintain cardiorespiratory fitness.

Balance practice

Balance-focused activities can help challenge stability and coordination.

Mobility and flexibility

Gentle mobility exercises can help maintain the range of motion required for everyday activities.

The biggest lesson is not that everyone over 65 needs to pass seven fitness tests.

It is that physical capability deserves attention as we age.

Strength, balance, endurance, and mobility affect how easily people can perform ordinary tasks and maintain independence.

The study provides evidence that these capabilities are also associated with mortality outcomes in older adults.

But the research should not be turned into a prediction tool.

A lower test score does not mean someone is destined to experience poor health.

Likewise, a high score does not guarantee a long life.
